Our Services

We will come to your home for a thorough assessment of your current situation, including support you may need inside and outside the home.  Initial assessment includes a 2-3 hour in home initial assessment and a one hour follow up visit to share your care plan, resources and recommendations.

Clients who benefit from this ongoing case management service need an initial care plan followed by monthly check-ins to discuss concerns and make plans for intervention as needed.  

Clients who use this service benefit from having a case manager for weekly contact, attending care conferences, coordinating with medical providers and managing family relationships and supports.

This service is for clients who have experienced a crisis - a fall, stroke, or sudden onset dementia, for example - and are in need are in need of immediate, temporary intensive support for care coordination, planning and family involvement.  We work with hospitals, care facilities and all involved parties to make a plan for crisis management and aftercare.
